I have reviewed the file and all documents and copies, and here is my
decision.

Your DAC report will be changed to reflect the following information:

Not eligible for Rehire (no change)
Resigned/Quit           (changed from Discharged to this)
Violation of DOT drug/
alcohol regulations in
the last 2 years                (changed from yes to no)

In reviewing the file contents no wrong doing on our part.  We documented
that the clinic confirmed you had not been there plus you were not
willing/able to provide a copy of the COC at the time.  Given the same
situation again, I don't see how we would or could change any of our
actions as at the time, we acted within the DOT guidelines with the
limited information and cooperation we received.

The changes on the DAC report reflect that we did indeed accept your
resignation.  I have faxed these changes to DAC and now consider this
issue closed.
